Practical questions
A headache during sex: when it needs emergency care
A first headache triggered by sexual activity needs medical assessment; a sudden explosive headache requires emergency care. Do not call it a harmless orgasm headache because it happened at a particular moment. Primary sexual headache is a diagnosis considered only after important secondary causes have been excluded.
General information for adults. Not a substitute for medical assessment.

Recognise the urgent pattern
Stop activity and seek emergency help for a sudden severe headache, particularly one reaching maximum intensity very quickly. New weakness, trouble speaking, collapse, confusion, visual change or significant neck symptoms add concern; do not wait for all of them. The International Headache Society states that first sexual-activity headache requires exclusion of serious causes such as bleeding, arterial dissection and reversible cerebral vasoconstriction syndrome.

Describe onset precisely to the treating team
Say that the pain began during sexual activity, whether it built gradually or appeared abruptly, and whether it was before or at orgasm. Include previous similar episodes, exertional headaches, medicines and substances, and any neurological symptoms. Embarrassment should not replace this useful history with a vague description of stress. Do not recreate the headache to test it or continue activity while waiting for a routine reply.

Use a follow-up plan only after assessment
If a clinician concludes that a primary sexual headache is likely, ask what findings support that conclusion, which investigations were needed and when recurrence should prompt reassessment. Some serious causes are not resolved by comparing your symptoms with an online description. Request individual advice about returning to activity; this guide gives no preventive medicine regimen, exercise challenge or assurance that a previous benign episode makes every later episode safe.

Common questions
Can the timing at orgasm prove it is harmless?
No. Serious causes can also occur with sexual activity and must be assessed appropriately.
Should I wait if this is my first explosive headache?
No. A sudden severe headache needs emergency assessment, not a routine sexual-health booking.
Can I restart a previous prevention medicine myself?
Do not use an old prescription to bypass assessment of a new or changed headache pattern.

Content policy and review statusQuestions before arranging an appointment?
Enquire about the next steps and availability. An enquiry does not confirm a booking or diagnosis and is not an emergency service.
